COVID-19 Vaccine Appointment FAQs

Do I meet the criteria to get vaccinated?

All Georgians 6 months of age and older qualify for the COVID-19 vaccine.

How do I make an appointment for a vaccine?

No appointments are required; however, scheduling an appointment will help reduce wait time. To schedule an appointment, call 770-904-3717. You can find additional information about the COVID-19 vaccine and scheduling by visiting https://www.gnrhealth.com/covid-vaccine-scheduling/.

What should I bring to my appointment?

Please bring your photo ID and insurance card (if applicable).  If you do not have photo ID, any piece of mail with your name and Georgia address will work. If you do not have insurance, the vaccine will still be at no cost to you. You will not be turned away if you do not have insurance.

Do I have to live in one of the counties to get vaccinated?

No. Anyone can get vaccinated at any of our vaccination sites.

Are there other options to get vaccinated?

Yes. Other providers, like doctors and pharmacies, in our communities are registering and working to get vaccine. To see a complete list of which providers are providing vaccine please visit: https://dph.georgia.gov/covid-vaccine

I didn’t get a CDC card at my visit. How do I get one?

If you still need your second dose, you will receive the card at your next appointment. If you have completed your immunizations series and did not receive a card at that time, you can visit the Lawrenceville Health Center, Norcross Health Center, Rockdale Health Center, or the Newton Health Center with your “Proof of Vaccination” paperwork, and one of our staff will fill out the card at no cost to you.

If you have lost your “Proof of Vaccination” paperwork or vaccination card, you can visit one of the health centers listed above to get a new card at no cost. Our staff must be able to retrieve your information from the GRITS system to fill out a new card for you. If you were vaccinated by a different provider, you may need to return to your original vaccine provider if they did not upload your record into GRITS.

(Please note this only applies to COVID-19 vaccines. If you need vaccine documentation for other vaccines, you will incur a fee.)
