Get Vaccine Records

Please complete the form below to request an electronic copy of you or your child’s vaccine records online. Parents or legal guardians can request their children’s vaccine records for children aged 17 or younger.

The records will be reported from the Georgia Registry of Immunization Transactions and Services (GRITS). Please note if you or your child received vaccines outside of Georgia and/or the location where you received your vaccines did not enter them in GRITS, your record may be incomplete. You can contact the practice where you received vaccines to request those records.

This service is available for residents of Gwinnett, Newton, and Rockdale Counties. If you live in another county (including outside of Georgia), please click here to get records from the state’s website.

Requests will be processed within 5–7 days. Immunization records and required school forms can be obtained by walking into any of our Health Department clinic locations. Please note that school forms are only available through our Health Department clinics and are not available through this online process.

I also understand that immunization registry information is confidential and will not be released to third parties without proper documentation.

If the requester is a social services agency, healthcare facility, or non-biological parent, you may be required to submit additional supporting documentation, such as a signed medical release, court orders, birth certificates, guardianship/custody documents, etc., as applicable. The health department will inform you if additional documentation is required.
