GNR Public Health and its community partners invite Gwinnett, Newton, and Rockdale county communities to participate in our Community Health Assessment survey, which is underway now through March 31.

By participating, you’ll help us evaluate current programs and identify community needs so the health department and its partners can work toward providing the services and programs that meet them.

The survey takes about 10-12 minutes to complete. You may stop at any time to submit your answers. Individuals who complete the entire survey may also enter for a chance to win one of 40 $25 gift cards.
