  • Government issued ID
  • Student ID

To qualify for a fee discount you must provide proof of income (not required for patients 19 years and younger).

ACCEPTED PROOF OF INCOME

  • W-2 Form
  • Current tax return
  • Current pay check / pay stub
  • Unemployment compensation notice or check stub
  • Notarized statement of income from employer on letterhead

Patients who do not provide the necessary proof of income will be expected to pay full fees at the time of visit.

Family Planning

Planning your family is essential to the well-being of mothers, babies, and the community at large.

 

 

 

Our Family Planning program offers health care services designed to provide women the support they need with planning when to have children, how to reduce unintended pregnancies, determine effective birth control methods and improve the well-being of families in our community.

 

Things to Know

Family Planning provides services that help prevent unintended pregnancies and sexually transmitted diseases (STD’s).We offer:

  • Gyn Exams, Pap Tests, Breast Exams
  • Screenings for Anemia, Diabetes and Cholesterol
  • Sexually Transmitted Disease (STD) and HIV screening, testing and treatment
  • Pregnancy testing* as both a walk-in service and through the Family Planning program
  • Preconception health counseling and pregnancy planning
  • Birth control services
  • We also provide education on the importance of maintaining a healthy lifestyle, along with needed immunizations.

 

*Pregnancy tests include a full pregnancy panel and case management to ensure follow-up treatment and testing is completed. Tests received with a full pregnacy panel include:

  • HIV
  • Hepatitis
  • Gonorrhea
  • Chlamydia
  • Syphilis

 

Most importantly, all services are safe and confidential and available to you at your local Gwinnett, Newton, or Rockdale County Health Center.
